The New York City Department of Health and Mental Hygiene maintain District Public Health Offices in the Bronx, Manhattan, and Brooklyn that are tasked with various community health initiatives. One of those initiatives is the Newborn Home Visiting Program (NHVP). In NHVP, community health workers visit the homes of new mothers in certain neighborhoods of Brooklyn and Manhattan.

The current paper-based solution has many areas of concern. Capturing and translating records from paper to software is time consuming and occasionally results in lost or inaccurate record keeping. Historical information may not be readily available to field workers, preventing access to prior interaction records and making it difficult to assess whether previous recommendations had been implemented.

The DOHMH leveraged the AccessPoint System to deploy the home visit solution for the Newborn program and help improve the health of NYC's new born children and mothers.
